FAMILY SUPPORT COORDINATOR JOB DESCRIPTION

The Family & Intercultural Resource Center (FIRC) is a community based non-profit which has been serving Summit County, CO families since 1993 with a mission to support family stability and self-sufficiency. Family Support Coordinators use a strengths-based approach to assess family needs and barriers, support family goal setting, and make appropriate resource referral. In order to improve education and economic outcomes for both parent and child, Coordinator will use a two generation (2Gen) approach to support family development. To improve family mental health, Coordinator provides mental health systems navigation and case management. Extensive training provided. This is a full-time position funded through a multiyear grant called the 2GO Pilot Program. GENERAL RESPONSABILITIES OF THE POSITION INCLUDE, BUT ARE NOT LIMITED TO:

  • Contact families referred from Summit County Human Services and other community partners to enroll them in the voluntary 2Gen Family Development Program
  • Provide strengths-based support and use a 2gen approach to enrolled families through in person coaching
  • Assess family needs and provide ongoing case management and appropriate support, under the supervision of the Program Manager and in collaboration with FIRC’s Family Support team
  • Provide one-on-one coaching to families related to postsecondary and employment development, and financial health and literacy
  • Assess client’s mental health and substance use, and provide care coordination with mental health and medical providers as necessary.
  • Create, implement, adjust and track family goals
  • Assist with coordination and/or facilitation of Family Support Workshops
  • Meet grant expectations and report deadlines
  • Complete and maintain all required documentation regarding referrals, assessments, and progress of families through the program in a timely and comprehensive manner
  • Enter weekly data in a timely manner
  • Represent the FIRC and the 2GO program at various community events or meetings
  • Participate in collaborative efforts between FIRC and the Workforce Center and FIRC and Colorado Mountain College
  • Attend and successfully complete all trainings required by the 2GO Grant and FIRC.

  • Ability to use a 2Gen and strengths-based approach when working with families
  • Ability to develop trusting relationships with families
  • Ability to take initiative and identify and develop systems within a program
  • Knowledge of community resources in order to make appropriate referrals
  • Ability to collect and manage data in a complete and timely manner
  • Ability to set and manage an independent schedule
  • Ability to work in a team and communicate with honesty, integrity, and a sensitivity to the needs and differences of diverse cultures
  • Ability to maintain client confidentiality at all times
  • Flexibility to be placed in different office locations

· Bachelor’s degree or higher in Social Work, Human Services, or related field

· Case Management experience in Social Work, Family Development, or related field for at least 2 years

· English and Spanish proficiency
